Renal Dosing Adjustment

Definition

Modification of drug dose or dosing interval for patients with impaired kidney function. Since many drugs are eliminated by the kidneys, reduced kidney function (measured by GFR or creatinine clearance) leads to drug accumulation and potential toxicity without appropriate dose reduction.

Example

Gabapentin dosing is reduced from 300 mg three times daily to 300 mg once daily in patients with a GFR of 15-29 mL/min.
