The gradual adjustment of a drug dose, typically starting low and increasing incrementally until the desired therapeutic effect is achieved with acceptable side effects. The 'start low, go slow' approach is especially important for drugs with narrow therapeutic indices.

Metformin is typically titrated: starting at 500 mg once daily, then increasing to 500 mg twice daily after 1-2 weeks, up to a maximum of 2,000-2,550 mg daily.
